Need to take a leave of absence?
We understand that life sometimes gets in the way of work. You may need to take time off from your work for a medical reason, to welcome a new child to your family, or to take care of an ailing family member.
Notify the leave administrator
After submitting your claim, notify the leave administrator at 1-800-852-2805 (option #2) or email firstname.lastname@example.org.
There are a few instances (i.e. personal non-medical or non-FMLA-related leave) where you would not need to open a claim but you’ll want to discuss with the leaves administrator prior to your leave starting.
